When a process is double click or highlighted to start a new case for a particular process a blank page is display instead of the dynaform but case is created as draft. Even for draft when double click the same blank page appears; this is not happening for other processes. See error image below:

Probably there is something wrong in a trigger which you fire before dynaform loads. Check your steps menu and form before.
If there is nothing there, create a new process and add task, form and assignment only. See if it is working.
