Instead of using Regenerate Data Report button, try to create the same report table with a different name. See if the new table is created with the same exact rows as the process case number. If everything is ok, than you can drop your first table and recreate it again.

Probably there is something wrong in a trigger which you fire before dynaform loads. Check your steps menu and form before.
If there is nothing there, create a new process and add task, form and assignment only. See if it is working.

jhacket, all component values are saved automatically to case variables when you submit (dont forget to add a submit button to the form). You can use the same variable at another form.
